Please include E-MAIL addresses for both the vendor contact AND for each vendor exhibitor. If you need to make CHANGES to the registration, DO NOT re-register! Instead, send an e-mail to christine_leyva@eri.eisai.com. When you register, you will receive an e-mail confirmation. THIS IS YOUR INVOICE! Then, send a copy with payment to the Division Treasurer, Bob Kowalski at Wyeth Research, Research Information, 150 Cambridgepark Drive, Cambridge, MA 02140, Tel: 617-665-6156, e-mail: bkowalski@wyeth.com. The cancellation policy will take effect. If a cancellation occurs, the parties agree that the following charges will apply. Registrants agree to the refund schedule below:

Part of your sponsorship is your company's name listing in the Spring 2006 Meeting Program. Send your company logo (black & white, small .gif, .jpg or .bmp file) and a short description (200 words or less) of your products/services to christine_leyva@eri.eisai.com by February 10, 2006. Platinum sponsors ALSO get a one page color ad in the Spring 2006 Meeting Program. Send an 8"x11" .gif, .jpg, or .pdf file to Christine M. Leyva by February 10, 2006. |

| HOTEL | The Peabody Hotel is providing SLA with a special hotel room rate for all Spring Meeting attendees and vendors: $150.00/night for March 23-31, 2006. Call the Peabody at 1-800-PEABODY and mention "Special Libraries Association." The Peabody Hotel has extended our special meeting rate of $150/night. The new deadline for room reservations at this rate is now Fri., March 10th. | ||||||||
| FREIGHT |
The basic shipping rates which are provided by the Drayage company at the Peabody Hotel are: $20.00 PER 100 POUNDS WITH A $40.00 MINIMUM CHARGE. Please download the form to make arrangements. Remember, Drayage must be used for ALL inbound shipments. Regarding outbound shipments, it is at your own discretion. The form is to be faxed to 901-259-2213 (FAX). | EXHIBITION |
Exhibit set-up is Sunday, March 26 from 11 AM - 4 PM. Set-up must be completed by 4 PM in time for the Sunday Evening Gala Reception. Exhibit breakdown is NO EARLIER than 2 PM on Tuesday, March 28. No pop-up booths allowed; table-top displays only. Tables are about 8 feet wide. Each VENDOR registration receives ONLY 1 TABLE. High-speed internet connection for 3 days (Sunday, Monday & Tuesday) and 1 power strip is $175.00 per exhibitor. Electricity alone is $75.00 for the 3 days. Though booth accessories are payable to the hotel as well as arranged directly with the hotel, you must indicate your accessory needs on the PH&T Vendor Registration form.
